It's been a year since Joe Carroll's death and after three men wearing Joe Carroll masks attack 6 people on a New York City subway car, the FBI asks Ryan Hardy to help them out with the case. He begs off saying he has no further interest in the case - but in fact, has been conducting his own investigation with the help of his niece, Max, an NYPD detective. Although several have been arrested, there are still 8 of Carroll's known followers still on the loose including Carlos Perez who Ryan manages to track down. The survivor from the attack on the subway, Lily Gray, doesn't have much to add to what is already known. Tensions continue to rise among the followers.
A year has passed since Joe's supposed death and we're taken back to when the female FBI agent was buried alive and Claire stabbed to death. Joe seems to becoming to grips with these terrible tragedies, yet another round of horror is about to begin.

Kevin Bacon seems to have really aged during the year. He is soon thrust into quite a situation when a band of crackpots wearing Joe masks invade a train and proceed to kill everyone except one woman in a subway car. Imagine the mayhem when the train pulls into the station.

The woman who survived the attack appears to be a patron of the arts. She is mysterious which leads me to be suspicious of her, yet she will be victimized again in the following episode.

There are two guys who look alike but are as eerie as they come. Murder appears to be their pleasure in life.
